   • Mobile number

   • Email address
   • Date of birth
   • Android user - you will see today’s date. Click on the year at the top, should be a scroll down menu. Choose the year you were born. Go to main calendar. Click on the month and date. Click set.
   • iPhone user - click on month, click on > (arrow). Scroll down to your year. Okay. Click set.
   • N.I. No (National insurance number)
   • Eligible to work in the UK – yes or no
   • Medical issues – insert medication details or type none
   • White collar role - click on your job role. Choose from the list
   • Or Blue collar role - click on your job role. Choose from the list
2. Click next. Sign document

Insert

1. Full name
2. Relation to you i.e. mother, father, wife, husband
3. Mobile number, next of kins mobile no
4. Landline number, next of kins landline number
5. Email address, next of kins email address

Click on the following

1. Gender
2. Ethnicity (BAME)
3. Are you currently unemployed? If yes, insert how many months or years?
4. Swipe category that applies to you.

Click next. Sign document

1. Do you have any unspent criminal convictions? Swipe yes or no. If yes, insert details
2. Click next. Sign document

1. Contact name

5. Swipe if answer is yes

3. Save in camera roll on your phone, add to document

5. Date, insert today’s date, press set. Next
6. Swipe yes where it says by ticking this box etc...

9. Red box will change to green to show it is complete. Needs to be 100%

5. Swipe yes to consent to the company processing your data
